Your Initial Visit:

Your first visit will include a consultation with a doctor that will listen to your complaints. After the consultation, a complete orthopedic/physical exam of your problem area will be performed by the doctor. The information you provide in the consultation combined with the findings of the exam will help determine if x-rays will be taken. (not all new patients have to have x-rays)

After the consultation, exam and possible x-rays, you and the doctor will decide what type of treatment would work best for you. If chiropractic is part of that care, you will most likely be treated on your first visit. Plan on spending about 45 minutes in the office for your first visit.

For your convenience, you can download and print your initial paperwork by clicking on patient forms. After completing and signing them, simply bring them with you on your first visit.

If you are coming in due to a motor vehicle collision, please also complete all three auto accident patient forms.

KIDS? A children's area is in the front office so that while you're being seen by the doctor they will be in a safe and supervised place with lots of toys!
