Some people joke that once you start seeing a chiropractor, you will become “addicted” to adjustments and have to keep going forever. Others worry that repeated adjustments will somehow make the spine dependent on treatment. There is also another concern behind this question: if you start getting adjusted, will you eventually feel the need to crack your own neck throughout the day, similar to someone who habitually pops their knuckles?
Chiropractic adjustments are not considered addictive in the medical sense, and there are no established withdrawal symptoms or physical dependence when spinal manipulation is stopped. However, there are several reasons someone may feel as though they want another adjustment, and those reasons are worth separating from addiction, habitual joint popping, and appropriate ongoing care.
Why Can an Adjustment Feel So Good?
Spinal manipulation can temporarily reduce pain, improve movement, and change how the nervous system processes pain and movement-related signals. Research suggests that some of the immediate effects of manipulation may come from changes in how the nervous system processes pain rather than simply moving a vertebra back into place. One controlled study also found a short-term increase in beta-endorphin levels following neck manipulation, suggesting that the body’s natural pain-relieving chemicals may contribute to the relief or sense of well-being some people experience after an adjustment.
For someone whose neck or back feels stiff or painful, the improvement after an adjustment may be noticeable. If those symptoms return later, it is natural to want the treatment that previously helped. That is not necessarily addiction. It may simply mean that the treatment provides relief of symptoms, much like massage, stretching, exercise, heat, or other treatments that someone may use repeatedly because they help.
Does Your Spine Become Dependent on Chiropractic Adjustments?
There is no good evidence that receiving chiropractic adjustments causes the spine to become weak, unstable, or incapable of functioning normally without continued manipulation. An adjustment also does not permanently “put the spine back into place.” The effects of spinal manipulation appear to involve a combination of changes in the joints and nervous system, many of which are temporary.
If stiffness or pain eventually returns, that does not necessarily mean the body has become dependent on chiropractic treatment. Some effects of an adjustment may be temporary, and symptoms can also fluctuate or recur because of ongoing contributing factors. These may include prolonged sitting, repetitive work, physical stress, previous injury, weakness, reduced activity, poor sleep, or simply the tendency of many muscle and joint problems to recur.
What About Feeling Like You “Need” to Be Adjusted?
A person can become accustomed to almost any treatment or activity that reliably makes them feel better. Some patients describe being able to tell when they “need an adjustment,” but what they are usually noticing is the return of stiffness, discomfort, restricted movement, or another familiar symptom. That does not necessarily mean that a joint has moved out of place or that the body has become dependent on treatment.
There can also be a behavioral component. Someone who becomes anxious whenever the neck or back feels slightly different and immediately believes an adjustment is required may become overly reliant on treatment for reassurance. Good chiropractic care should not encourage that type of fear. Patients should generally become more confident in their ability to move, exercise, work, and manage occasional symptoms, not increasingly afraid of what will happen if they miss an adjustment.
Do You Have to Keep Going to a Chiropractor Forever?
No. For an uncomplicated muscle or joint problem, the purpose of treatment is usually to improve pain and function and then reduce treatment as the patient improves. At our clinic, successfully performing an adjustment is not itself considered a successful outcome. The outcome that matters is whether the patient is getting better.
As recovery progresses, normal activity, exercise, rehabilitation, and self-management should generally become increasingly important. If someone is doing well without treatment, there is no requirement to continue getting adjusted simply because they previously received chiropractic care.
Is Maintenance Chiropractic Care Different From Addiction?
Yes. Some patients have recurring or persistent muscle and joint problems and find that occasional treatment helps control episodes or maintain function. This is commonly called maintenance care or supportive care, and there is some research supporting this approach in selected patients.
In a randomized clinical trial involving people with recurrent or persistent low back pain who had responded well to an initial course of chiropractic treatment, patients assigned to scheduled maintenance care experienced approximately 13 fewer days of bothersome low back pain during the following year compared with patients who returned for treatment when symptoms recurred. The maintenance group received an average of fewer than two additional chiropractic visits during that year. A later systematic review concluded that maintenance care may be reasonable for selected patients with recurring low back pain who previously responded well to chiropractic treatment.
That does not mean everyone should receive maintenance adjustments. Someone who functions normally without treatment may have little reason to continue care. Conversely, if symptoms or difficulty with normal activities consistently worsen when treatment is withdrawn and improve again when intermittent treatment resumes, occasional care may provide a meaningful benefit. The important question is not how long someone has been seeing a chiropractor, but whether the treatment is still producing a meaningful benefit.
Is Neck Popping the Same as a Chiropractic Adjustment?
No. A pop or crack is usually associated with joint cavitation—a rapid pressure change inside a joint that can create a small gas cavity and produce a popping sound. It can occur during normal movement, stretching, cracking your own joints, or a professionally performed chiropractic adjustment. The sound itself is not the treatment.
A chiropractic adjustment, or chiropractic manipulative treatment (CMT), is a procedure performed by a trained clinician after evaluating the patient and deciding whether manipulation is appropriate. Depending on the technique, the chiropractor applies a controlled force, direction, and amount of movement to the spine or another joint. Some adjustments produce an audible pop and others do not, and an adjustment does not have to make a noise to be effective. A systematic review evaluating audible popping during spinal manipulation found no evidence that treatments accompanied by a pop produced better pain outcomes than treatments without one.
Likewise, hearing several pops does not necessarily mean that several vertebrae were successfully “put back into place.” The sound represents cavitation occurring within one or more joints, not proof that a particular bone was repositioned. This is one reason repeatedly cracking your own neck is not equivalent to receiving chiropractic treatment.
When someone cracks or manipulates their own neck, the goal is usually to reproduce the sensation or sound of a pop, often by rotating, bending, or pulling on the neck until a joint pops. Chiropractic manipulation, by contrast, is a treatment performed by a clinician rather than an attempt merely to produce a cracking sound. Developing a habit of repeatedly twisting or pulling on your own neck throughout the day is therefore a separate behavior from receiving chiropractic care.
Will Chiropractic Make Me Start Cracking My Own Neck?
Another common meaning behind the question about becoming “addicted” to chiropractic is more practical: If I start getting my neck adjusted, will I eventually feel compelled to crack my own neck throughout the day?
There is no evidence showing that receiving chiropractic adjustments causes someone to develop a habit of repeatedly cracking or “popping” the neck. Some people do develop habits of cracking their neck, back, fingers, or other joints because doing so temporarily relieves a sensation of stiffness or tension. Knuckle cracking, for example, can become habitual, and research has found a small temporary increase in joint range of motion immediately after a knuckle is cracked. People who repeatedly crack their own neck may be responding to a similar sensation.
That is different from physical dependence or addiction, and it is also different from receiving a chiropractic adjustment. There is generally no reason that receiving professional chiropractic treatment should require someone to manipulate their own neck between visits. If someone feels compelled to forcefully crack the neck multiple times every day because it repeatedly feels stiff or uncomfortable, it may be more useful to determine why the neck continually feels that way rather than simply chasing the popping sensation.
Is It Bad to Crack Your Knuckles Every Day?
Probably not, at least with respect to arthritis. The idea that cracking your knuckles causes arthritis has been repeated for generations, but research has not supported it. In a study of 215 adults, researchers compared people with and without hand osteoarthritis seen on X-rays. A history of habitual knuckle cracking was not associated with a higher rate of osteoarthritis, even when researchers considered how frequently the joints were cracked and how many years the person had been doing it. An earlier study involving 300 adults likewise found no higher rate of hand arthritis among habitual knuckle crackers.
Based on the available evidence, regular knuckle cracking does not appear to cause osteoarthritis. The cracking sound itself usually represents joint cavitation—a rapid pressure change inside the joint associated with formation of a small gas cavity. It does not mean that bones are grinding together or that cartilage is being damaged each time the joint pops.
Does Cracking Your Neck Every Day Cause Arthritis?
There is much less direct research on habitual neck cracking than on knuckle cracking. There is currently no good evidence demonstrating that routinely popping the neck causes arthritis in the neck, but there also are not comparable long-term studies of habitual neck cracking that allow us to make the same strong conclusion that we can for the fingers.
Arthritis and degenerative disc changes in the neck are extremely common with aging and are influenced by many factors, including genetics, previous injury, repetitive or heavy physical demands at work, smoking, and normal age-related changes. Hearing or producing a pop in the neck does not by itself mean that the joint is being damaged.
Deliberately and forcefully manipulating your own neck is nevertheless different from casually cracking a knuckle. The neck contains the spinal cord, nerve roots, and important blood vessels, including the vertebral and carotid arteries. Someone attempting to make the neck pop may repeatedly use substantial twisting or movement near the joint’s limit. Published case reports have described complications involving the nerves, spinal cord, or blood vessels following forceful self-manipulation of the neck. A 2026 systematic review identified 24 published cases, but case reports cannot determine how frequently such complications actually occur.
For that reason, occasional painless popping that occurs during normal neck movement is different from repeatedly twisting or forcefully manipulating the neck in an effort to make it crack. If you find yourself needing to forcefully crack your neck many times every day to relieve persistent stiffness or discomfort, the better question may not be whether the popping itself is causing arthritis. It may be why the neck keeps becoming uncomfortable enough that you feel the need to do it.
When Should Ongoing Chiropractic Treatment Be Reconsidered?
Ongoing treatment deserves reconsideration when there is no measurable improvement in pain, movement, function, activity tolerance, or another meaningful outcome. Simply repeating the same treatment indefinitely despite a lack of benefit makes little sense. Depending on the situation, the treatment plan may need to change, exercise or rehabilitation may need greater emphasis, additional testing or evaluation may be appropriate, or referral to another healthcare provider may be indicated.
Treatment also should not be based on frightening a patient into believing that the spine will deteriorate, become permanently misaligned, or become damaged if adjustments are stopped. A patient should be able to make an informed decision about continuing treatment based on what the care actually accomplishes.
The Bottom Line
Chiropractic adjustments are not known to cause addiction, physical dependence, or withdrawal symptoms, and receiving chiropractic care does not make the spine weak or dependent on manipulation. Some people enjoy the immediate reduction in stiffness or discomfort and may want treatment again when those symptoms return. Others with recurrent conditions may obtain a measurable benefit from occasional supportive or maintenance care. Neither situation, by itself, means the person is addicted to chiropractic adjustments.
Receiving chiropractic care also does not appear to cause people to become habitual self-neck-crackers. Knuckle cracking has not been shown to cause osteoarthritis, while considerably less research exists on habitual neck cracking. Occasional painless neck popping is not the same thing as repeatedly and forcefully manipulating your own neck, and neither should be confused with professionally performed chiropractic manipulation.
The goal should be appropriate use of treatment, not treatment for its own sake. If chiropractic care is helping, that benefit should be identifiable. If it is no longer helping, the treatment plan should be reconsidered.
