Rear-end collisions are among the most common types of motor vehicle accidents. Although many are described as minor crashes, even a relatively low-speed impact can expose occupants to sudden acceleration and deceleration forces.
These forces may affect the muscles, ligaments, joints, discs, nerves, and other tissues throughout the neck, back, shoulders, and pelvis. A collision can also cause a concussion without the head directly striking the steering wheel, window, or another object.
Some injuries cause immediate symptoms. Others become noticeable over the following hours or days.
Understanding the injuries commonly associated with rear-end collisions can help you recognize symptoms and determine when medical evaluation is appropriate.
Why Do Rear-End Collisions Cause Injuries?
When a vehicle is struck from behind, the seatback rapidly accelerates the occupant's torso forward.
Because of inertia, the head initially lags behind the torso. The neck first moves into extension relative to the torso and then rebounds into flexion. This rapid acceleration-deceleration sequence can place substantial stress on the cervical spine and surrounding tissues.
The seatbelt and properly positioned head restraint help limit occupant movement, but they cannot eliminate all forces transmitted through the body. The impact may also affect the:
- Thoracic spine
- Lumbar spine
- Shoulders
- Chest wall
- Sacroiliac joints
- Brain
The likelihood and severity of injury depend on multiple factors, including the relative size and speed of the vehicles, seat design, head-restraint position, occupant posture, direction of impact, use of restraints, and individual health factors.
Common Injuries After a Rear-End Collision
Whiplash-Associated Disorders
Whiplash-associated disorder is the condition most commonly associated with rear-end collisions.
The term whiplash-associated disorder, or WAD, describes the collection of symptoms that can develop after rapid acceleration and deceleration of the neck. It is not limited to a single injured structure.
Affected structures may include:
- Neck muscles
- Spinal ligaments
- Facet joints and joint capsules
- Fascia
- Intervertebral discs
- Nearby nerves
Symptoms may include:
- Neck pain
- Neck stiffness
- Reduced range of motion
- Headaches
- Shoulder or upper-back pain
- Dizziness
- Numbness or tingling in an arm
Mid-Back and Low-Back Injuries
Although the neck receives considerable attention after a rear-end collision, the thoracic and lumbar spine may also be injured.
The torso may be loaded against the seatback, restrained by the seatbelt during rebound, or twisted during the collision. Muscles may also contract forcefully when an occupant braces for the impact.
Possible injuries include:
- Thoracic or lumbar muscle strains
- Ligament sprains
- Facet joint irritation
- Sacroiliac joint irritation
- Disc injuries
- Nerve irritation
Symptoms may include:
- Mid-back pain
- Low-back pain
- Muscle spasms
- Pain between the shoulder blades
- Difficulty bending or twisting
- Pain when sitting or standing
- Pain radiating into a leg
Headaches
Headaches commonly occur after rear-end collisions and may have more than one possible cause.
Cervicogenic headaches originate from injured or irritated structures in the neck. They often begin near the base of the skull and may spread toward the forehead, temple, or area behind the eye.
Other possible causes include:
- Neck muscle tension
- Whiplash-associated disorders
- Facet joint irritation
- Temporomandibular joint irritation
- Concussion
A severe, worsening, or unusual headache should be evaluated promptly.
Concussion
A concussion is a mild traumatic brain injury that can result from a blow to the head or from a hit to the body that causes the head and brain to move rapidly.
Direct contact between the head and an object is not required.
Possible concussion symptoms include:
- Headache
- Dizziness
- Nausea
- Balance problems
- Memory problems
- Difficulty concentrating
- Sensitivity to light or sound
- Fatigue
- Sleep disturbances
- Feeling mentally slowed or foggy
Not every post-accident headache or episode of dizziness is caused by a concussion. However, suspected concussion symptoms require appropriate medical evaluation.
Shoulder and Seatbelt-Related Injuries
The shoulder may be injured when the seatbelt restrains the torso or when an occupant grips the steering wheel and braces for impact.
Possible injuries include:
- Shoulder muscle strains
- Ligament sprains
- Rotator cuff injuries
- Joint irritation
- Chest-wall bruising or soreness
Persistent shoulder weakness, loss of motion, significant swelling, or difficulty lifting the arm should be evaluated.
Severe chest pain, abdominal pain, or difficulty breathing requires urgent medical attention.
Disc and Nerve Injuries
The discs between the vertebrae help absorb force and permit spinal movement. A collision may injure a disc or aggravate a preexisting disc condition.
Possible injuries and related conditions include:
- Disc bulges
- Disc herniations
- Annular fissures
- Nerve-root irritation
Symptoms suggesting nerve involvement may include:
- Pain radiating into an arm or leg
- Numbness
- Tingling
- Burning pain
- Muscle weakness
- Changes in reflexes
Disc bulges and degenerative changes are also common in people who do not have pain. An MRI finding by itself does not establish whether a disc abnormality was caused by the collision.
Imaging findings should be interpreted together with the accident history, symptom onset, physical examination, neurological findings, and prior imaging when available.
Sacroiliac Joint and Pelvic Injuries
The sacroiliac joints connect the sacrum at the base of the spine to the pelvis.
A rear-end collision may load these joints through compression, twisting, or asymmetric bracing. Symptoms may include:
- Pain near one side of the lower back
- Buttock pain
- Pain near the back of the hip
- Pain when walking
- Pain when changing positions
- Difficulty standing on one leg
Because several lumbar, hip, and pelvic conditions can produce similar symptoms, a physical examination is needed to help identify the source of pain.
Why Are Symptoms Sometimes Delayed?
Some people feel relatively normal immediately after a collision but develop pain, stiffness, headaches, or dizziness later.
The body's stress response may temporarily alter pain perception during and immediately after an accident. In addition, muscle guarding, tissue irritation, and inflammatory processes may develop over time.
Concussion symptoms may also appear or become more noticeable hours or days after the injury.
Delayed symptoms do not necessarily indicate a severe injury, but new, persistent, or worsening symptoms should not be ignored.
When Is Emergency Medical Care Necessary?
Call 911 or seek emergency medical care for symptoms such as:
- Loss of consciousness
- Increasing confusion or unusual drowsiness
- A severe or worsening headache
- Repeated vomiting
- Seizures
- Slurred speech
- One pupil appearing larger than the other
- New weakness, numbness, or loss of coordination
- Severe neck or back pain
- Loss of bladder or bowel control
- Numbness in the groin or saddle area
- Chest pain
- Difficulty breathing
- Significant abdominal pain
These symptoms may indicate a brain injury, spinal injury, internal injury, or another condition requiring immediate evaluation.
When Should You Schedule an Evaluation?
A nonemergency evaluation is appropriate when you experience:
- Persistent neck or back pain
- Increasing stiffness
- Reduced range of motion
- Recurring headaches
- Dizziness
- Shoulder pain
- Pain radiating into an arm or leg
- Numbness or tingling
- Weakness
- Symptoms interfering with sleep, work, or normal activities
An evaluation can help identify the likely source of the symptoms and determine whether imaging, treatment, or referral is needed.
How Are Rear-End Collision Injuries Diagnosed?
Diagnosis begins with a detailed history and physical examination.
Your healthcare provider may ask about:
- How the collision occurred
- The direction of impact
- Your position in the vehicle
- Seatbelt and head-restraint use
- Whether you were bracing for impact
- When each symptom began
- Previous injuries or health conditions
The examination may include an assessment of:
- Range of motion
- Areas of tenderness
- Joint mobility
- Muscle strength
- Reflexes
- Sensation
- Balance and coordination
- Orthopedic and neurological findings
Imaging is not automatically necessary after every rear-end collision. The type of imaging should be selected according to the examination findings and level of concern for a specific injury.
X-Rays
X-rays may help identify:
- Certain fractures
- Spinal alignment
- Arthritic or degenerative changes
- Other bone abnormalities
Normal X-rays do not rule out muscle, ligament, disc, nerve, or concussion-related injuries.
CT Scan
CT provides more detailed images of bone than standard X-rays and is commonly used when there is substantial concern for an acute spinal fracture, skull fracture, or bleeding within the skull.
MRI
MRI may be recommended when symptoms or examination findings suggest:
- Spinal cord involvement
- Nerve-root compression
- Disc injury
- Ligament injury
- Other significant soft-tissue abnormalities
MRI is not routinely required for every patient with uncomplicated neck or back pain.
Treatment Options
Treatment depends on the structures involved, the severity of the symptoms, neurological findings, and whether other injuries have been excluded.
Chiropractic Care
When chiropractic treatment is appropriate and conditions requiring emergency or other medical management have been ruled out or addressed, chiropractic care may be used for selected musculoskeletal injuries.
Treatment may include:
- Spinal or extremity manipulation
- Joint mobilization
- Soft-tissue treatment
- Range-of-motion exercises
- Progressive rehabilitation
- Activity recommendations
The techniques used should be modified according to the patient's condition, comfort, and stage of recovery.
Massage Therapy
Following appropriate clinical evaluation and referral or prescription, massage therapy may be incorporated into the treatment of appropriate soft-tissue injuries.
Potential goals include:
- Reducing muscle tension
- Decreasing soreness and stiffness
- Improving comfortable movement
- Supporting a gradual return to activity
Massage therapy does not replace medical evaluation for concussion, fracture, neurological injury, or internal injuries.
Therapeutic Exercise
A gradual exercise program may help restore:
- Range of motion
- Strength
- Flexibility
- Spinal and shoulder stability
- Coordination
- Functional movement
Prolonged immobilization is generally avoided unless it is specifically required for an identified injury.
Medical and Specialist Care
Depending on the diagnosis, additional care may include:
- Medication
- Physical therapy
- Advanced imaging
- Neurological evaluation
- Orthopedic consultation
- Pain-management procedures
- Surgical consultation
Coordination among providers may be necessary when several injuries are present.
Can Rear-End Collision Injuries Become Chronic?
Yes. Although many people improve within several weeks, some continue to experience symptoms for months or longer.
Persistent problems may include:
- Chronic neck pain
- Chronic back pain
- Recurring headaches
- Reduced mobility
- Dizziness
- Sleep disturbance
- Difficulty working or performing daily activities
Persistent symptoms do not always mean that additional structural damage is occurring. Recovery may be influenced by the original injury, pain severity, neurological symptoms, sleep disruption, activity limitations, previous health conditions, and other health and recovery-related factors.
